The task force identified multiple decision points that, if handled differently, could have prevented the gunman from firing shots at the rally. Failures to secure high-risk areas, lack of clear guidance to local partners, and inadequate manpower were among the key issues outlined in the report. The Secret Service personnel with limited experience in planning roles were also criticized for not clearly understanding their responsibilities. Overall, the task force concluded that the failures leading to the events in Butler were due to preexisting issues in leadership and training within the Secret Service.
